It is SNU policy that all undergraduate students carrying twelve or more hours of coursework must be covered by health insurance and provide a proof of insurance coverage annually.
If you already have a health insurance plan, i.e. from an employer, parent, spouse/partner, etc…, make sure that insurance plan provides coverage within a 50 mile radius of Sierra Nevada University. Some insurance plans (such as Kaiser or Nevada Health Plan) require that you visit their own network providers to receive care. That network may not be available within a reachable distance from SNU, which is where you will be spending most of your time while attending school.
Proof of Insurance Requirements
Provide your proof by completing the Proof of Coverage form below. All students attending SNU for the fall semester must provide proof of health insurance coverage before August 1. All students entering in the spring semester must provide proof of health insurance coverage before January 1.
Proof of health insurance coverage must be submitted by the stated deadline every year that the student is enrolled at SNU. The form must be completed accurately with the proper insurance card attached to be accepted.
A hold will be placed on the account of all undergraduate students carrying twelve or more hours of coursework who have not submitted valid proof. The hold will be removed once the proof of coverage has been received. This hold will prevent you from adding or dropping courses, viewing your grades, receiving transcripts, or graduating.
Students who have not yet provided proof of insurance also
- will not be able to participate in ANY off-campus school activities
- will not receive roommate assignments if living on campus

- Scan or photograph both sides of the student’s insurance card, and attach the image(s) of the insurance card to the form. Feel free to upload legible cell phone photos.
