Students must have health insurance coverage to participate in off-campus activities. Submit the Student Health Insurance Waiver (link above) prior to August 1st to participate.
I understand that this Off-Campus Activity is a school trip, sponsored by Sierra Nevada College (SNC), and that all school rules are in effect. I agree to abide by school rules and any special conditions as set down for this trip including, but not limited to, curfew, group assignments, and security of individual and group belongings. In the event deems my behavior unruly or subordinate, and I am found in violation of the trip’s rules, I will be sent back to Sierra Nevada College at my personal expense. Although, the decision to send a participant home is rare, I understand that the trip leaders will make the final decision on my fate as a participant in the activity. I understand that I will be personally responsible for my return travel expenses and will be charged by Sierra Nevada College for any expenses incurred.
As a participant, I understand that the use of alcohol, illegal drugs, and certain prescription drugs are prohibited during the duration of this Off-Campus Activity. Failure to abide by this alcohol and drug policy will result in expulsion from the trip without refund and possible loss of course credit. I understand that I will become personally responsible for my return travel expenses.
I understand that I will be partaking in possible rigorous activities, and that there is a risk of damage or loss to my personal belongings. I understand that I am fully responsible for my personal belongings and will not hold Sierra Nevada College or the trip leaders liable for any expenses due to their loss and/or damages.
I understand that if I do not own the equipment needed to participate in an Off-Campus Trip, I have the option to rent equipment for my trip from the SNC Outdoor Adventure Program, and will become personally responsible for its condition and whereabouts during the contracted time in my custody. I understand that I will be charged late fees, repair fees, and/or replacement fees of the equipment if it is lost, damaged, or stolen during the contracted time it is issued to me, or if I fail to return it by the contracted return date.
I have verified that I have no past or current physical or psychological condition that might affect my participation in the Off-Campus Activity, other than as described on the Medical History in this form. I am able to participate without causing harm to myself, or to others. The medical information given to SNC is accurate and all pertinent medical conditions have been disclosed. I understand that SNC’s admission of me to the Off-Campus Trip is not intended as a representation that SNC trip leaders will be able to manage successfully a medical event or emergency related to a disclosed, or undisclosed, medical condition. The responsibility for determining a participant’s suitability for a course is not SNC’s but, rather, the participant’s, guided by family and her or his physician. SNC reserves the right to refuse admission or remove a participant from a course for any reason it deems in the best interests of the student or the school.
I understand that this trip may involve risks, dangers, and hazards. These risks, dangers, and hazards many include, but are not limited to, natural disasters, weather conditions, varying steep terrain, natural and man-made obstacles and structures, equipment failure, collisions with objects and or other people, and exceeding one’s abilities. I understand that injuries of all types are a potential occurrence in off-campus activities. I understand that the trip leaders cannot guarantee my safety in these activities. As a participant in this trip, I take full responsibility and agree not to hold Sierra Nevada College or the trip leaders, in the normal execution of their duties, liable for any accident or injury that could ensue, or be related to participation on this trip.
SNC is authorized to obtain or provide emergency hospitalization, surgical or other medical care for me. I understand that situations may arise in which third- party medical care is not available and which require SNC staff and faculty to provide first aid and possibly more advanced procedures, employing wilderness first responder training. Any third-party medical care provider is authorized to exchange pertinent medical information with SNC. Costs reasonably associated with medical services, including evacuation, shall be borne by me.
As a participant, I acknowledge that SNC may share my contact information with other SNC students or graduates and/or with environmental organizations or strategic partners in which SNC graduates may be interested; and SNC may use my student’s name, contact information and photo or other image in promotional materials and press releases.
Activities and Risk
I understand that some SNC Off-Campus Activities live, camp and travel out of doors. Activities vary from trip to trip and include, among others, hiking and backpacking through mountainous and other terrain, mountaineering and climbing on rock cliffs, steep snow, whitewater kayaking/rafting, sea kayaking, skiing, snowboarding, and canyoneering. I further understand and acknowledge that the activities of the programs have risks, including certain risks, which are inherent. Inherent risks are those, which cannot be eliminated without destroying the unique character of the activities. The same elements that contribute to the unique character of these activities can cause loss or damage to equipment, accidental injury, illness, or, in extreme cases, permanent trauma, disability or death. The following describes some, but not all, of the inherent risks of SNC off-campus activities:
Acknowledgement/Assumption of Inherent and Other Risks
As a participant, I acknowledge that I have read and understand the Student Agreement Letter and all pertinent information regarding this Off-Campus Activity, including but not limited to, SNC Catalog, SNC Student Handbook, specific course descriptions/syllabi, trip itineraries, etc.
I acknowledge that I have read the description of Activities and Risks contained herein and am aware that SNC Off-Campus activities can be dangerous and include many risks (listed but not limited to those above). The staff and faculty of SNC has been available to more fully explain the nature and physical demands in which I will be engaged, and certain inherent and other risks of the proposed trip and I accept them.
Agreement of Release and Indemnity
I hereby release, waive, indemnify and agree not to sue Sierra Nevada College for all or any liability to me for any and all losses, damages, injuries, or any claim or demand on account of any injury to my person or property, or on account of my death resulting from any cause, including negligence of SNC staff or hired guides, while I am participating in an SNC Off-Campus Trip. I expressly acknowledge and agree that outdoor adventure can be dangerous and involves serious and unpredictable risks of bodily injury, property damage, and death that I intend the foregoing waiver and release of SNC to be as broad and inclusive as permitted by law.
I further agree, as a participant on this Off-Campus Trip, as follows:
Any dispute between me and SNC will be governed by the substantive laws (not including the laws which might apply the laws of another jurisdiction) of the State of Nevada, and any mediation or suit shall occur or be filed only in the State of Nevada.
If any part of this agreement is found by a court or other appropriate authority to be invalid, the remainder of the agreement nevertheless will be in full force and effect.
THE PARTICIPANT HAS READ THIS DOCUMENT AND UNDERSTANDS AND VOLUNTARILY AGREES TO ITS TERMS, WHICH SHALL BE BINDING UPON THEM, THEIR HEIRS, ESTATE, EXECUTORS AND ADMINISTRATORS.
